Medical Forms (REQUIRED)
PLEASE DO NOT SEND SMARTPHONE PICTURES OF THESE DOCUMENTS.
Please follow instructions closely as to when each of these forms needs to be submitted. Forms must be completed and either mailed, faxed, or emailed as soon as possible following registration.
Ray Reid Soccer School Medical Release Form
DUE DATE: Within TWO WEEKS after Registration
This form must be completed and signed by the child's physician. Forms from past completed physicals are generally acceptable, however they must:
- Include a copy of the physician's handwritten signature (electronically signed forms will not be accepted).
- Be based on a physical exam within the last 2 years (from the start date of your applicable camp session).
- Include the child's vaccination records, with dates of administration. Check boxes or "up to date" will not be accepted.
- State the overall wellness of the child and ability to participate in sports activities without any restrictions.
Ray Reid Soccer School Authorization to Administer Medication Form (Required for prescription AND over the counter medications)
DUE DATE: First Day of Camp (Please Do Not Mail to Us)
This form must be signed by both your child's physician and a parent in order for the camp to administer any medication to your child. PLEASE BE AWARE THAT WE CANNOT ADMINISTER PRESCRIPTION OR OVER-THE-COUNTER MEDICATION (i.e. Advil/Tylenol, Claritin, multivitamins, acne medication, etc.) WITHOUT THIS FORM BEING SIGNED BY A DOCTOR. Per Connecticut state law all over the counter medications MUST be administered by a nurse/doctor during camp hours. Do not mail this form to us - bring it with you on the first day of camp, signed, with any medications your child may require (please label all medications accordingly).
Drop-Off/Pick-Up Forms (If Necessary)
PLEASE NOTE PICK-UPS/DROP-OFFS MUST OCCUR DURING THE HOURS OF 7:30AM - 10:00PM
DUE DATE: Before July 1st
This form must be completed and signed by the child's parent/guardian if they intend on arriving late to the first day of their applicable session of camp. Check-in for camp sessions generally takes place between 10:00AM and 2:00PM depending on your session of camp (times subject to change, please consult your final camp details when your start dates approaches).
DUE DATE: Before July 1st
This form must be completed and signed by the child's parent/guardian if they intend on picking-up their player early OR they will be picked up by someone other than a legal parent/guardian (regardless of time) on the final day of their applicable session of camp. Camp sessions generally begin check-out around 11:30AM depending on your session of camp (time subject to change, please consult your final camp details when your start dates approaches).
