This form must be completed by the child's physician and mailed or faxed (860.674.1704) into the address as soon as possible following registration. A physical form issued by the state of Connecticut within the last 2 years is acceptable. No camper will be allowed to participate without mailing or faxing in the signed form before camp begins. If you have attended the RRSS in the past we do not keep the physical forms, you will need to provide us with a new one!

This form must be signed by both your child's physician and a parent in order for the camp to administer medication to your child. PLEASE BE AWARE THAT WE CANNOT PROVIDE MEDICINE - EVEN OVER-THE-COUNTER DRUGS WITHOUT THIS FORM BEING SIGNED BY A DOCTOR. Do not mail this form to us - bring it with you on the first day of camp, signed, with any medications your child may require (please label all medications accordingly)
